About the Practice
If you've been told that leaking when you run is "just part of being a mom," or that pelvic pain after pregnancy is something you have to live with — Momumental Physical Therapy exists to prove otherwise. Founded by Dr. Lisa Smiley, a Doctor of Physical Therapy and certified Pregnancy and Postpartum Athleticism Coach in Indianapolis, Momumental helps women stay strong and active through every stage of motherhood, from pregnancy through postpartum and beyond.
What sets Momumental apart is a fitness-forward approach to pelvic health. Dr. Smiley doesn't treat the pelvic floor in isolation — and you won't spend your sessions lying on a table doing kegels the whole time. Her practice operates inside a personal training facility, giving her access to real gym equipment so she can assess and treat you with the actual movements causing your symptoms. Leaking during deadlifts? Pain with running? She'll watch it happen in real time and build a plan around it.
Dr. Smiley's background as a former Division I swimmer at the University of Arkansas shapes her approach. That athletic lens, combined with specialized training in pelvic health and obstetrics, means you're getting a clinician who sees motherhood as the athletic event it truly is. She's passionate about getting women comfortable with strength training, knowing it's one of the best things you can do for long-term pelvic health.
Your first visit is a 90-minute, one-on-one evaluation. Dr. Smiley will dig into your health history, talk through your goals, and take you through a full-body movement screen. If an internal pelvic floor assessment is warranted, that's available — but it's never the default, and it's always your choice. You'll leave with a clear picture of what's going on and exercises to start working on at home. You'll never be double-booked, handed off to an aide, or rushed.
The goal at Momumental is to get you back to doing what you love without fear, leaking, or limitation. Whether that means returning to CrossFit postpartum, training for a triathlon, or picking up your toddler without pain, Dr. Smiley builds a plan that gets you there — and equips you with the knowledge to maintain your results long after your last visit.

What to expect
Your first visit is a 90-minute, one-on-one evaluation with Dr. Smiley. She'll review your history, discuss your goals, and perform a full-body movement assessment. An internal pelvic floor exam is included only if needed and with your consent. Sessions take place in a gym setting with access to real equipment. Momumental is a cash-based practice with virtual sessions available.
